女性高危型人乳头瘤病毒感染2501例调查

摘要: 目的了解广州地区2013年和2014年女性高危型HPV感染情况,对该地区宫颈癌的预防和疫苗的开发提供指导。方法
采用荧光PCR分型定性检测方法,对2501例来院就诊的女性患者或体检者宫颈分泌物中的13种高危型HPV特异性DNA核酸
片段进行检测。结果2013年和2014年高危型HPV总感染率分别为14.85%(146/983)、14.56%(221/1518),差异无统计学意义
(χ2=0.041,P=0.839)。各年内各年龄段感染率差异亦无统计学意义(χ2=2.916,P=0.572;χ2=6.494,P=0.165)。2013年和2014年
HPV亚型的总体构成比差异无统计学意义(χ2=11.872,P=0.452),2013年各亚型感染频次构成比从高到低依次为HPV52、16、
58、56、39、51、68、59、31、35、18、33、45型,而2014年为HPV52、16、58、68、18、51、56、39、31、33、59、35、45型。结论广州地区女
性高危型HPV感染率较高,各年龄段女性均应定期进行高危型HPV筛查,以预防和早期发现宫颈癌。开发新的多价疫苗或通
用疫苗预防HPV感染,从而降低宫颈癌的发病率。

Abstract: Objective To survey the prevalence of high-risk human papillomavirus (HPV) in woman in Guangzhou during the
period from 2013 to 2014. Methods A total of 2501 women in Guangzhou seeking medical attention in our hospital underwent
high-risk HPV genotype screening of cervical specimens using real-time PCR. Results The prevalence of high-risk HPV
infection among the women was 14.85% (146/983) in the year 2013, similar to the rate of 14.56% (221/1518) in 2014 (χ2=0.041, P=
0.839); no significant differences were found in the high-risk HPV infection rates between different age groups in either
2013 (χ2=2.916, P=0.572) or 2014 (χ2=6.494, P=0.165). The constituent ratio of the 13 types of high-risk HPV showed no
significant difference between 2013 and 2014 (χ2=11.872, P=0.452). The 13 HPV genotypes detected, listed in a descending order
of the constituent ratios, included HPV-52, -16, -58, -56, -39, -51, -68, -59, -31, -35, -18, -33 and -45 in 2013, and were
HPV-52, -16, -58, -68, -18, -51, -56, -39, -31, -33, -59, -35 and-45 in 2014. Conclusion We report a high prevalence of high-risk
HPV among women in Guangzhou, which suggests the necessity of screening for high-risk HPV-DNA among women at all
ages for prevention and early detection of cervical cancer.
