rFVIIa 成功控制合并肾衰呼吸衰竭的消化道大出血1例

摘要： 目的探讨消化道大出血的治疗。方法重组人凝血因子VIIa（rFVIIa）是一种凝血因子制剂，国外文献报道可控制危及生
命的出血。1例78岁的消化道大出血患者合并肾衰呼吸衰竭，经过常规药物治疗无效，出血持续，出现休克。应用rFVIIa20 μg/kg
静脉注射治疗。结果出血迅速控制，生命体征逐渐稳定。结论对于常规方法无法控制的消化道大出血，rFVIIa可作为尝试治疗。

Abstract: Recombinant activated factor VII (rFVIIa) is a novel therapeutic agent for life-threatening massive gastrointestinal
bleeding. We report a case of massive gastrointestinal bleeding in a 78-year-old female patient with respiratory and renal failure.
After failure of management of the bleeding with routine pharmacotherapy, we gave the patient rFVIIa injection at the
dose of 20 μg/kg and the bleeding was rapidly controlled. Adverse side effects of the drug were not observed in this patient.
