一种肾盂成形术的腹腔镜训练模型

A training model for laparoscopic dismembered pyeloplasty

摘要: 目的介绍一种离断性肾盂成形术的腹腔镜训练模型。方法采用猪肾、鸡食道嗉囊制作成肾盂输尿管连接部狭窄模型,
鸡躯干用于固定该模型。在自制腹腔镜模拟训练箱内,对8名完成腹腔镜基本技能培训但无实际腹腔镜手术经验的年轻医师
进行10轮的模拟训练,记录每次手术的完成时间,并根据每次手术术中缝合撕裂针数,术后检查吻合口针距、边距的情况,累计
成手术的失误评分,对吻合口质量进行初步评估。结果8名年轻医师均成功完成了培训,手术时间从最初72.70±8.26 min降至
最后的54.50±7.69 min;吻合口失误评分从最初5.60±1.35分降至最后的2.50±1.58分,差异均有统计学意义(P<0.05)。结论此
模型制作简易、术野逼真、训练有效,能较好地模拟腹腔镜离断性肾盂成形术的各个手术步骤,适合于临床医师进行该术式的初
级培训。

Abstract: Objective To introduce a training model for laparoscopic dismembered pyeloplasty. Methods The model of
ureteropelvic junction obstruction was simulated using pig kidney and chicken crop. With the model placed in a training box,
8 residents without laparoscopic experience but with basic technical training performed 10 rounds of anastomosis according to
the standard operation procedures. For each resident, the operating time and quality of the operation were recorded and
assessed. Results All the 8 residents successfully completed the training. The operating time decreased from 72.70±8.26 min in
the first training session to 54.50±7.69 min in the last training session, and the anastomosis error score decreased from 5.60±1.35
to 2.50 ± 1.58 min, showing significant differences between the recordings (P<0.05). Conclusion This simple training model
allows effective simulation of the surgical procedures of laparoscopic pyeloplasty and is suitable for training surgeons without
laparoscopic experiences.
