慢性阻塞性肺疾病病人肺内小动脉对血栓素及内皮素的反应性降低

摘要: 目的探讨肺通气功能正常与慢性阻塞性肺疾病(COPD)病人肺内小动脉对血管活性物质(如:血栓素A2、内皮素等)的反
应性及其可能的机制。方法分离人肺内小动脉,制备血管条,采用微血管张力测定技术,分别比较肺通气功能正常与COPD病
人肺内小动脉对60 mmol/L高钾溶液、血栓素A2类似物U46619(100 nmmol/L)、内皮素(ET-1)(30 nmol/L)诱导反应的张力变
化;观察用非特异性环氧化酶抑制剂吲哚美辛孵育后肺内小动脉张力变化幅度的差异。结果与肺通气功能正常病人相比,
COPD病人肺内小动脉对U46619(对照组:7.68±2.31 mN,COPD组:5.89±2.43 mN,P<0.05)和ET-1(对照组:5.97±2.08 mN,
COPD组:3.99±2.15 mN,P<0.01)引起的张力明显下降,而两组对高钾溶液引起的张力变化无明显差异(对照组:5.65±1.70 mN,
COPD组:5.02±1.38 mN,P>0.05);吲哚美辛孵育后,对照组肺血管张力下降(对照组:孵育前平均张力比孵育后平均下降39%,
P<0.01;COPD组:孵育前平均张力比孵育后平均上升5%,P>0.05)。结论慢性阻塞性肺疾病病人对血栓素类似物U46619、内
皮素ET-1反应性降低。

Abstract: Objective To investigate the reactivity of intrapulmonary arterial rings to vasoactive substances as thromboxane A2
and endothelin-1 in patients with chronic obstructive pulmonary disease (COPD). Methods Intrapulmonary arterial rings
isolated from patients with normal lung function and COPD were mounted in a Multi Myograph system to determine the
reactivity of the intrapulmonary arterial rings to 60 mmol/L KCl, thromboxane A2 analogue U46619 and endothelin-1 before
and after preconditioning with the COX synthase inhibitor indomethacin. Results The reactivity of intrapulmonary arterial
rings to U46619 and endothelin-1 was significantly decreased in patients with COPD. The reactivity to U46619 was
dramatically decreased in patients with normal lung function after application of indomethacin. Conclusion The reactivity of
intrapulmonary arterial rings is significantly decreased in patients with COPD.
