双重滤过血浆成分分离法用于ABO血型主要不合非亲缘骨髓移植

Application of double-filtration plasmapheresis in major ABO-incompatible unrelated bone marrow transplantation

摘要: 目的 观察双重滤过血浆成分分离法在ABO血型主要不合非亲缘骨髓移植中的疗效。方法 采用双重滤过血浆成分分离法去除体内血型抗体,预防5例ABO血型主要不合非亲缘骨髓移植并发溶血反应和纯红再障。结果 受者一次性双重滤过后,全部患者血型抗体均≦1:16,输注血型主要不合骨髓过程中仅1例发生轻度短暂急性溶血反应,无一例发生迟发性溶血反应和纯红再障,全部植入成功。结论 双重滤过血浆成分分离法可有效预防ABO血型主要不合骨髓移植并发溶血反应和纯红再障。

Abstract: Objective To evaluate the effect of double-filtration plasmapheresis (DFPP) in major ABO-incompatible unrelated allogenic bone marrow transplantation. Methods The red cell antibodies were removed by DFPP in 5 recipients to prevent hemolysis and pure red cell aplasia following major ABO-incompatible unrelated bone marrow transplantation. Results The isohemagglutinin titers of the 5 recipients were below 1:16 after DFPP with only one recipient developed transient mild acute hemolysis. Delayed hemolysis and pure red cell aplasia occurred in none of the cases and all recipients were completely engrafted. Conclusion DFPP can effectively prevent hemolysis and pure red cell aplasia in recipients of unrelated bone marrow transplantation with major ABO-incompatibility.
