急性心功能不全动物改进模型的建立

Establishment of a modified canine model of acute cardiac insufficiency

摘要: 目的 探讨用结扎冠状动脉前降支(LAD)后快速右室心内膜起搏的方法尝试建立急性心功能不全动物模型的可行性。方法 完全结扎犬前降支和进行快速右室起搏,使心输出量(CO)较基础状态稳定地下降25%和50%,分别测定基础及心输出量下降状态下的血压(AP)、血氧(SaO2)、平均右房压(mRAP)、平均肺毛压(mPCWP)、系统血管阻力(SVR)值并以超声心动图测定心腔大小和左室射血分数(LVEF)。结果 结扎LAD和快速右室起搏后CO较基础状态均稳定地下降25%和50%,CO降低25%和50%时,mRAP、mPCWP、SVR均显著升高,AP、SaO2均显著降低;心脏各腔室明显扩大,LVEF显著降低。结论 结扎冠状动脉前降支及快速右心室起搏可成功制作急性心功能不全的动物模型。

Abstract: Objective To establish a canine model of acute cardiac insufficiency (ACI) by coronary artery occlusion and right ventricular pacing. Methods Twelve dogs were subjected to rapid ventricular pacing after ligation of the left anterior descending coronary artery (LAD), to induce the cardiac output (CO) to reduce by 25% and 50% from the basal level. The arterial pressure (AP), arterial oxygen saturation (SaO2), mean right atrial pressure (mRAP), mean pulmonary capillary wedge pressure (mPCWP), and systemic vascular resistance (SVR) were measured in the dogs according to different CO conditions. Echocardiography was used to measure the cardiac size and left ventricular ejection fraction (LVEF). Results CO was decreased by 25% and 50% steadily from the basal level after right ventricular pacing following ligation of the LAD. AP and SaO2 decreased while mRAP, mPCWP, and SVR increased significantly following CO reduction. The cardiac cavity became larger while LVEF decreased after CO was decreased by 25% and 50%. Conclusion Canine model of ACI can be successfully established by right ventricular pacing following ligation of the LAD.
