两样本孟德尔随机化方法分析血液代谢物与冠心病的因果关系

摘要:

目的 利用两样本孟德尔随机化方法探究血液中的代谢物与冠心病发病风险之间的因果关系。方法 采用公共数据库中的大规模的代谢组-全基因组关联分析数据和冠心病全基因组关联分析数据,通过逆方差加权法和其他4种两样本孟德尔随机化模型来评估血液代谢物与冠心病之间的因果关系。并进行异质性检验、基因多效性检验和敏感性分析来评估结果的稳定性和可靠性。结果 486个代谢物中,共有32个代谢物与冠心病的因果关系效应值达到名义上显著(逆方差加权法,P<0.05),其中包含11个已知代谢物和21个未知代谢物。有3个已知代谢物[N-乙酰鸟氨酸,(Des-Arg9)-缓激肽,丁二酰基肉碱]在至少3种孟德尔随机化模型中达到统计学显著。利用leave-one-out法进行敏感性分析并剔除混杂的工具变量后,3个代谢物对应的因果效应不再具有统计学意义。结论 在本研究中纳入的486个代谢物,尚未有充足证据表明其与冠心病发生风险之间存在稳健的因果关系。

Abstract:

Objective To explore the causal relationship between blood metabolites and the risk of coronary artery disease (CAD) using a two-sample Mendelian randomization (MR) approach. Methods Based on the data from a large-scale metabolome-based genome-wide association study (mGWAS) and the GWAS of CAD, we investigated the causality between blood metabolites and CAD using an inverse variance weighted (IVW) method and another 4 two-sample MR models. Heterogeneity, horizontal pleiotropy, and sensitivity tests were performed to evaluate the stability and reliability of the results. Results Among the 486 blood metabolites, 32 metabolites showed nominally causative association with CAD with the IVW method (P<0.05), including 11 known metabolites and 21 unknown metabolites. Three known metabolites [N-acetylornithine, bradykinin-des-arg(9), and succinylcarnitine] were statistically significant in at least 3 MR models, but their causal effects on CAD were no longer significant after sensitivity analysis using leave-one-out method and elimination of the confounding instrumental variables. Conclusion There is no strong evidence to support a robust causal relationship between the 486 blood metabolites and the risk of CAD.
